The Collaborative partnered with Compassion Health Toledo to repurpose a former public library facility into a new Primary Care health clinic and pharmacy. The project involves the build out of a new 9,000sf clinic and community space within the 10,000sf former Mott Library building.
Programming for the facility includes:
- Waiting Space
- Care Team Station
- Exam and Exam / Procedure Rooms
- Clean and Soiled Rooms
- Toilet Room
- Draw Lab
- Charting Space
- Conference Room
- Offices for Financial Counselor; ED / COO; Community Health Workers; and Enabling / Social Worker
- Phone Room
- Supply and Donation Storage
- Dining, Kitchenette and Lockers
- Community Space / Future Dental Space
